Use the form below to order your Fall Festival shirt. Note that the form will automatically calculate the correct total cost of your order. Please make sure to send in your check or cash payment before the 28th to guarantee your order!

Please select “women’s cut” or long sleeve if you’d prefer these styles where available. There is an additional $2 per shirt for either of these options. Please note that women’s cut is not available for the long sleeve style.
